Hello LMHA U15AA Parents and Players,
All players, coaches, and volunteers MUST please fill out the COVID ASSESSMENT FORM link below the day of and prior to the scheduled activity. I suggest bookmarking the form(s) that you need, so you can easily access them on your mobile device.
Players and Coaches must fill out the form for the division that they are in. Volunteers also need to fill out the form for the division they are volunteering with.
