Permits are required for ALL EXHIBITION GAMES that you are hosting.
IF THIS FORM IS COMPLETED INCORRECTLY - YOU CAN BE DENIED YOUR PERMIT.
Only the home team needs to apply for the permit. Once you have requested an Exhibition Permit, you will be contacted with your Permit number that must be written on the score sheet.
You MUST submit your score sheet to our zone rep: Daryl Pickering - disciplinecentral1@hockeyalberta.ca
